New Hope for Platinum-Resistant Small Cell Lung Cancer: Lurbinectedin Arrives at Jiahui

A novel alkylating agent that targets cancer transcription and reshapes the tumor microenvironment offers a new treatment path for patients with limited options after platinum-based therapy.

SHANGHAI — Jiahui International Cancer Center has officially introduced Lurbinectedin for injection (Zepzelca®), a globally approved novel alkylating antineoplastic agent, for adult patients with metastatic small cell lung cancer (SCLC) whose disease has progressed on or after platinum-based chemotherapy.

Approved by China's National Medical Products Administration (NMPA) in December 2024, lurbinectedin has already received international guideline recommendations. Its arrival at Jiahui marks a meaningful expansion of treatment options for one of the most challenging lung cancer subtypes.

"Lurbinectedin is now officially available at Jiahui International Cancer Center, bringing a new standard-of-care option to Chinese patients facing platinum-resistant or platinum-progressive small cell lung cancer."

The Challenge of Platinum-Resistant SCLC

Small cell lung cancer accounts for approximately 15% of all lung cancers. It is highly aggressive, progresses rapidly, and is frequently diagnosed at advanced stages. For patients whose disease worsens during or after platinum-containing chemotherapy, subsequent treatment options have historically been extremely limited, with poor prognosis and rapid deterioration.

Lurbinectedin's approved indication precisely targets this high-unmet-need population: adults with metastatic SCLC that has progressed on or after platinum chemotherapy.

A Dual Mechanism Against Tumor Growth

Lurbinectedin exerts antitumor activity through a distinctive dual mechanism:

  • Selective inhibition of oncogenic transcription: It binds to the DNA minor groove, blocking active transcription and leading to tumor cell apoptosis.
  • Modulation of the tumor microenvironment: It reduces tumor-associated macrophages and other supportive cells, weakening the tumor's defense network.

By disrupting both tumor cell proliferation and its surrounding protective environment, lurbinectedin offers a biologically rational treatment path for patients with relapsed or refractory SCLC.

Clinical Efficacy: Meaningful Responses and Manageable Safety

Lurbinectedin demonstrated encouraging efficacy and a controllable safety profile in the pivotal PM1183-B-005-14 trial and the China bridging study LY01017/CT-CHN-101:

Clinical Endpoint Result Clinical Interpretation
Objective Response Rate (ORR) 35.2% More than one-third of patients achieved significant tumor shrinkage
Median Overall Survival (mOS) 9.3 months Clinically meaningful survival extension in later-line treatment
Safety Profile Manageable Most common adverse events are myelosuppression, fatigue, and hepatotoxicity, generally manageable with supportive care

The approval of lurbinectedin signals a new era for SCLC treatment in China — the era of transcription-targeted therapy — offering renewed hope for patients who have exhausted first-line platinum options.
